You may be eligible for transition care if you are:
- A patient in a public or private hospital
- Have been told that you are ready to leave hospital
- Would benefit from receiving services for a brief period

What is a Home Care Package (HCP)?
A Home Care Package is the highest package that a client can be approved for. A client has one provider for all services. MAC - HCP - provides info regarding: eligibility, what is not included and what it is etc.
